Community Health Worker (SDoH) *Eastern Shore/Southern MD
About the role
This position will be part of the Maryland Physicians Care (MPC) Social Determinants of Health (SDoH) team. This team works directly with MPC members to address social barriers to optimal health. In conjunction with the SDoH team, this person will work closely with medical providers, primary care teams, and social services agencies to provide short-term care coordination and connection to resources and support to program members to improve their health and general well-being through education and provision of coordination of care and services. Works in both clinical and community-based settings.
Responsibilities
- Conduct intake interviews with members and complete the Social Determinants of Health (SDOH) assessment.
- Communicate the purpose of the program to the members and the impact it may have on their well-being.
- Help patients identify socio-economic issues that affect their overall health and develop health/social management plans and goals.
- Partner with community organizations, governmental agencies, and local resources to develop productive relationships and programs.
- Aid patients in understanding care plans and instructions.
- Assist members in accessing health-related services, and overcoming barriers to obtaining needed medical care/social services (scheduling appointments with social services agencies, and assisting with completion of applications for programs for which they may be eligible).
- Provide support and advocacy during medical visits or when necessary to ensure members' medical needs and referrals required are being conveyed.
- Follow up with both members and providers regarding health/social services.
- Maintain positive working relationships with members, providers, nurse case managers, agency representatives, and office staff, from diverse cultural and socio-economic backgrounds.
- Works to reduce cultural and socio-economic barriers between members and institutions.
Requirements
- Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services.
- Knowledge of transportation and other barriers to care that may be encountered by the member.
- High school diploma or GED.
- Possession of a valid Maryland State Driver’s License is required.
- 2 years of healthcare experience, preferably in the MD/DC area with Medicaid members.
Secondary Functions
- Observe confidentiality of member records in accordance with MPC policies and procedures.
- Must be available and willing to travel 50-75% around Eastern Shore/Southern Maryland.
