Community Health Worker

Key Responsibilities

• Conduct community outreach and engagement
• Build trust-based relationships with clients and families
• Conduct Social Determinants of Health (SDOH) screenings
• Provide care navigation and referrals
• Support maternal health and behavioral health coordination
• Conduct follow-up support and resource connection
• Collaborate with healthcare providers and community organizations
• Maintain accurate documentation and client records
• Participate in community events and outreach activities

Qualifications

• High school diploma or equivalent required
• Experience in community outreach, healthcare navigation, social services, peer support, or related work preferred
• Bilingual candidates strongly encouraged to apply
• Experience working with maternal health, behavioral health, or underserved communities is a plus

Required Training

All hired Community Health Workers are required to complete onboarding requirements and NJDOH-required Community Health Worker training, including enrollment in and/or completion of the Colette Lamothe-Galette Community Health Worker Institute (CLG-CHWI) training program during the grant period.
