Community Health Worker or Housing Transition Coordinator

About the role

This is an in-person, Monday-Friday role for candidates located in the Cincinnati, Ohio area and covering/traveling to Butler, Clermont, Clinton, Hamilton, Warren, Brown, Highland, Greene, Fayette, and Adams Counties.

Responsibilities

  • Serve as a liaison between healthcare and housing professionals, including service coordinators, discharge planners, and community-based service providers.
  • Maintain knowledge of all service lines: adult day health, personal care/private duty, service coordination, skilled care, and hospice.
  • Collaborate closely with individual case managers to arrange services that align with each consumer’s preferences and community resources, including physicians, pharmacies, and other essential providers.
  • Aid individuals in locating housing and obtaining necessary materials and supplies (i.e. social security card, ID, completing applications, etc.).
  • Support consumers in accessing entitled benefits and resources.
  • Provide exceptional customer service by addressing questions, concerns, and problems, ensuring daily needs are met.

Qualifications

  • Education: High school diploma or GED required. Experience minimum of 1 year in social service or healthcare (STNA) delivery with adults, elderly and family populations; 2 years' experience preferred.
  • Knowledge of supportive services and community resources in assigned areas/counties.
  • Knowledge of housing services in the areas preferred.

Travel

This Cincinnati, OH area role will travel frequently throughout the week to visit and serve clients in the following counties: Butler, Clermont, Clinton, Hamilton, Warren, Brown, Highland, Greene, Fayette, Adams and other assigned locations. This role is paid for travel time and offers reimbursement for mileage.

Licensure

Valid driver’s license, automobile insurance and compliance with National Church Residences motor vehicle policy is required.
