Community Health Worker position focused on coordinating care for members as part of an inter-disciplinary care team.

Responsibilities

  • Engage with members in various settings (hospital, provider office, community agency, member’s home, telephonic, or electronic communication) to establish effective, professional relationships.
  • Accompany members to appointments and other social service encounters when necessary.
  • Coordinate logistics to support members’ care plan goals and interventions, including reminders, transportation, and childcare arrangements.
  • Verify eligibility, enrollment history, demographics, and current health status of each member.
  • Contribute to assessments by gathering information from members, families, providers, and other stakeholders.
  • Assist in developing and implementing individualized care plans based on member needs and preferences, reporting to the Case Manager.
  • Identify and help manage barriers to achieving care plan goals.
  • Empower members to manage and improve their health, wellness, safety, adaptation, and self-care through care coordination.
  • Provide health education and wellness materials as directed by the Case Manager or Team Lead.
  • Evaluate member satisfaction through open communication and monitoring of concerns or issues.
  • Maintain appropriate documentation within Care Management program protocols and guidelines.
  • Identify process improvements to enhance members’ experience and share with leadership.
  • Travel regularly to conduct member, provider, and community-based visits to ensure effective program administration.
  • Perform other job duties as requested.

Requirements

  • High School Diploma or General Education Diploma (GED) required.
  • Minimum of two (2) years of experience in community settings with at-risk populations, providing coordination of services (volunteer or paid).
  • Community Health Worker Certification or equivalent approved training program preferred.
  • Valid driver’s license, vehicle, and verifiable insurance required. Employment conditional on successful clearance of a driver’s license record check and verified insurance.
  • Annual Influenza vaccination required (October 1 – March 31) as a condition of continued employment.
  • Must reside in the same territory assigned to work in; exceptions may be considered based on business need.

  • Proficient with Microsoft Office (Outlook, Word, Excel).

  • Ability to identify problems, opportunities, and communicate effectively with management.
  • Developing knowledge of local, state, and federal healthcare laws and company policies regarding case management.

  • Proven ability to improve processes to enhance service delivery.

Working Conditions

  • Mobile position requiring regular travel to different work locations, including homes, offices, or public settings.
  • Exposure to weather conditions typical of the location; may require standing or sitting for long periods.
  • May be required to travel greater than 50% of the time to perform work duties.
  • Use of general office equipment (telephone, photocopier, fax machine, personal computer).
  • Flexible hours, including possible evenings and/or weekends to serve member needs.

Pay

$35,900.00 - $57,300.00 annually. Salary is determined by a combination of education, training, experience, scope and complexity of the role, discretion required, and external/internal data. Bonus opportunities may be available based on company and individual performance.
