Community Health Worker - Mental Health

About the role

The Community Health Worker provides outreach and engagement with Managed Care Plan members, supporting care management through transportation assistance to appointments and social services.

Responsibilities

• Assisting clients in navigating healthcare and social services systems
• Transporting clients to appointments using personal or company vehicles
• Supporting individuals with mental health, physical health challenges, and social needs
• Promoting engagement and providing case management support

Qualifications and Skills

• High School Diploma or GED
• Minimum two years’ experience in mental health or intensive case management
• Experience supporting physical health challenges and navigating healthcare systems
• Valid driver’s license, auto insurance, and willingness to use personal or company vehicle for work
• CPR, CPI, First Aid certifications (or willingness to obtain within 60 days)
• All background checks, health screenings, and license verifications required prior to employment
