Community Health Worker (Maryland Corps/Service Year Option Member)

About the Role

TidalHealth is seeking a motivated Community Health Worker (CHW) through the Maryland Corps / Service Year Option. In this role, you will serve as a vital link between healthcare providers and the community, helping patients navigate the healthcare system, improve their health, and connect with essential community resources. You will join an interdisciplinary healthcare team of care coordinators, nurses, social workers, and medical providers. This position is designed as a learning opportunity with extensive structure, mentorship, step-by-step guidance, and ongoing coaching to help you build foundational workplace skills for your future career or education.

Responsibilities

• Build trusting relationships with patients and families to help coordinate care and meet health needs at home.
• Follow best practices to assess patient needs regarding transportation, utilities, housing, financial assistance, and health insurance.
• Help patients prioritize their goals and link them to primary care providers or external community support agencies.
• Provide culturally appropriate health information, give informal advice on chronic disease self-management, and advocate for community needs.
• Conduct home visits and meet patients in diverse settings. Perform basic direct services, such as blood pressure screenings.
• Maintain timely, accurate records of all activities in compliance with standards. Enter data into healthcare platforms like Epic.

Schedule

• Hours: Monday – Friday, day and/or evening shifts.
• Flexibility: Occasional work outside standard weekday hours may be required to meet patient needs.
• Setting: Office environments, community events, and patient homes.

Training & Requirements

What We Provide (On-the-Job Training)

• CHW Certification: We will provide the pathway and support for you to earn your Community Health Worker Certification within one (1) year of your hire date.
• Extensive Supervision: Expect daily mentorship, clear step-by-step instructions, and supportive skill-building activities as you transition into an independent workflow.

Eligibility & Background Check Requirements
