Community Health Worker Lead -- Pediatrics
About the Role
The Community Health Worker Lead plays a critical role in advancing health equity, improving health outcomes, and promoting the well-being of all patients by addressing health-related social needs and overcoming barriers to accessing care. Embedded within pediatric primary care, this role combines direct patient support with day-to-day leadership of the Community Health Worker team. The Lead collaborates closely with multidisciplinary care teams to support families with care coordination, assessment of unmet health-related social needs, and navigation to appropriate hospital and community resources.
In addition to maintaining a patient-facing caseload, the Lead provides guidance, mentorship, training, and operational support to Community Health Workers, ensuring high-quality, equitable, and family-centered care. The role serves as a resource to staff, promotes best practices in community-based care, assists with program coordination and workflow, and supports team operations, including work allocation and coverage in the absence of the Manager. While this is not a clinical role, it requires a strong understanding of or the ability to learn basic medical concepts, the ability to recognize when referral to a licensed clinician is appropriate, and a commitment to fostering the professional growth and success of the Community Health Worker team.
Responsibilities
- Collaborate with healthcare providers, community organizations, and other stakeholders to develop and implement effective strategies to improve health outcomes.
- Ensure compliance with regulatory requirements and program standards.
- Collect and analyze data to measure program effectiveness and identify opportunities for improvement.
- Assist families to apply for social services, including Medicaid or Women, Infants, and Children (WIC).
- Available for daily contact with CHWs to provide resource navigation peer support.
- Direct patients/families to additional resources as needed.
- Update and input progress notes, participant charts, and EHR according to organizational standards.
- Counsel clients in individual or group sessions to assist them in dealing with substance abuse, mental or physical illness, poverty, unemployment, or physical abuse.
- Counsel or aid family members to assist them in understanding, dealing with, or supporting the client or patient.
Requirements
- Bachelor's Degree in Public Health, Social Work, or a related field of study required. Experience in lieu of education may be considered.
- Minimum 3-5 years of experience in community outreach, health education, or a related field.
- Prior leadership experience strongly preferred.

- Strong organizational and project management skills.
- Basic computer skills, including Microsoft Office and database management.
