Community Health Worker I- Clinical Services

About the role

This position reports to the Director of Care Management and will function as an essential team member for Honor Community Health who advocates for the patient and helps to remove barriers to improved patient health. The Community Health Worker (CHW) engages in collaboration with multiple team members as part of an interdisciplinary team to deliver integrated, person-centered, and comprehensive care to eligible beneficiaries to successfully address the complexity of social determinants of health needs in the presence of other comorbid physical and behavioral health conditions.

Responsibilities

- Engage in collaboration with multiple team members as part of an interdisciplinary team to deliver integrated, person-centered, and comprehensive care. - Advocate for the patient and help to remove barriers to improved patient health. - Work with patients to identify and address social determinants of health needs. - Collaborate with other departments to ensure continuity of care. - Utilize CHAMPS, NextGen, and Microsoft Office to manage patient records and communications. - Assist patients with accessing necessary resources and services.

Requirements

- High school diploma or equivalent. - Current MiCHWA Community Health Worker certification or the ability to obtain within 12 months of hire. - 1+ year of experience working in a clinical environment. - Strong computer skills utilizing CHAMPS, NextGen, and Microsoft Office.
