Purpose Statement/Position Summary
The Community Health Worker, under the supervision of department leadership, provides specific services to patients and their families. Coordinates case management and social care services. Demonstrates commitment to the principles of family-centered care.
Minimum Qualifications/Work Experience
- Experience with community resource navigation, social-care support, and/or health education.
- Experience with public/government benefits through work or lived experiences, preferred.
- Bilingual skills, preferred.
Education/Licensure/Certification
- High school diploma/GED OR 4 years of related education and/or work experience.
