Community Health Worker (Hybrid)

Responsibilities

  • Perform in-person visits to members in hospitals, post-acute, rehabilitation and other healthcare and community facilities.
  • Participate in member engagement efforts, including in-person, calls, texts, and email campaigns.
  • Pre-screen patients for eligibility, educate patients about the benefits of participating in the program, and answer questions.
  • Build trusting relationships with patients and caregivers to encourage enrollment.
  • Maintain documentation in the Karoo Health EHR system for tracking and reporting purposes.
  • Drive patient enrollment by providing an overview of the Karoo Health program and Model of Care through patient education and interaction to ensure patients understand program services and benefits.
  • Coordinate with hospital/facility case management and discharge planning teams to understand discharge plans and support members successful transition to home or the next setting of care.
  • Meet monthly outreach, enrollment, and engagement goals.
  • Manage inbound calls from patients responding to inquiries about enrollment and ongoing engagement with the Karoo Care Program.
  • Communicate regularly with all members of the interdisciplinary team to share key information, coordinate tasks and provide support for the enrollment process.
  • Participate in team meetings, ongoing training, and quality improvement initiatives.
  • Special projects, initiatives, and other job duties as assigned.

Qualifications

  • High school graduate or equivalent.
  • Ability to work within hours of 8 am-5 pm Monday-Friday.
  • Community Health Worker/Partner, Certified Medical Assistant, LPN, or CNAs.
  • Credit-card-experience preferred.
  • Minimum three years of related experience in health care and/or hospital and/or physician clinic/practice setting.
  • Valid driver's license and proof of automobile insurance.
  • Ability to commute to hospitals and other healthcare facilities and possess reliable transportation.
  • General understanding of enrollment and billing processes and procedures.
  • Strong interpersonal skills and ability to quickly establish rapport with providers and patients.
  • Highly comfortable in group/team dynamics and able to fit in with clinic staff and teams.
  • Confidence in explaining new programs and initiatives to people/patients; ability to connect and engage with people comfortably.
  • Experience with team collaboration and coordination of care with external sources.
  • Exceptional communication skills and can practice active listening.
  • Strong written communication skills.
  • Comfort with ambiguity and a fast-paced culture; you can adapt and approach problems with a solution-oriented mindset.
