Community Health Worker (CHW)

About the role

The role is responsible for providing comprehensive healthcare services to patients in various departments including Adult & Family Care, Pediatrics, Women's Health, Prenatal Care, Family Planning, Chiropractic, Pain Management, Urgent Care, Dental, Behavioral Health, Pharmacy, Lab Services, and COVID & Flu.

Responsibilities

• Provide high-quality medical care to patients in a variety of settings.
• Conduct thorough patient assessments and develop treatment plans.
• Communicate effectively with patients, families, and other healthcare providers.
• Adhere to all healthcare policies and procedures.
• Maintain accurate and up-to-date patient records.

Requirements

• Current license to practice medicine in the state.
• Bachelor’s degree in a related field.
• At least 2 years of relevant work experience.
• Strong interpersonal and communication skills.
• Proficiency in computer systems and software.

Qualifications

• Excellent clinical judgment and problem-solving skills.
• Ability to work independently and as part of a team.
• Knowledge of healthcare regulations and compliance standards.
