Community Health Worker - Chuukese

Primary Purpose: Provide case management, outreach, health education, and interpretation services. Assist in training students in the health professions and health care professionals regarding cultural issues and health care. Serve as an advocate for appropriate, quality health and social services. This position is a member of the Patient Centered Health Care Home team, where each team member plays an integral role in providing patient-centered health care.

Responsibilities

• Provide oral and written interpretation services, including health education and counseling as needed.
• Assist patients in filling out intake and other forms for medical visits.
• Help patients connect with entitlements: My Connection referrals, medical insurance, financial assistance, housing, etc.
• Provide referrals to outside agencies as directed by the health care team.
• Document all patient contacts in the patient’s chart.
• Attend training and classes to improve services.
• Assist patients in connecting with appropriate staff for medication refills.
• Provide health education and counseling to patients.
• Assist in training students in health professions, health care professionals, and patients regarding cultural issues and health care.
• Assist in scheduling patient appointments and registering new patients, including obtaining necessary signatures (registration forms, sliding scale notifications, HIPAA, ROI, etc.).
• Call or write patient appointment reminders, no-show notifications, and referrals.
• Recall patients for follow-up appointments at least once a month.
• Verify patient insurance and assist with outside billing problems.
• Observe safety and security procedures, report unsafe conditions, and use equipment properly.
• Provide clerical support for various departments or projects.
• Assist in other delegated projects and clinical outreach activities.
• Attend trainings and classes to improve services.

Regular and reliable on-site attendance is required.

Requirements

• High School Diploma or GED required.
• Certification as a professional translator preferred but not required.
• Certification as a professional community health worker preferred but not required.
• Post-secondary education preferred but not required.
• One year of experience in providing bicultural and bilingual services in a community health center or similar setting preferred but not required.
• Formal training in medical terminology preferred but not required.
• Formal training as a community health worker preferred but not required.

Skills

• Knowledge of the community and community service resources, especially in the Kalihi-Palama and Honolulu Downtown areas, preferred.
• Fluency in English and Chuukese required; ability to read, understand, and translate technical/medical instructions in both languages.
• Basic computer skills.
