Community Health Worker- Behavioral Health Home

About the role

This position reports to the Director of Care Management and functions as an essential team member for Honor Community Health Home projects. The Community Health Worker (CHW) delivers integrated, person-centered, and comprehensive care to eligible beneficiaries.

Responsibilities

• Deliver six core services: comprehensive care management, care coordination, health promotion, comprehensive transitional care, individual and family support, and referral to community and social support services.
• Track and follow up on care details, ensuring they are documented in the Electronic Health Record and population health management tool.
• Communicate effectively, organize efficiently, manage time effectively, and multitask successfully.

Requirements

• A High School Diploma or equivalent.
• Current MiCHWA Community Health Worker certification or the ability to obtain within 12 months of hire.
• Two (2) years of experience directly related to the duties and responsibilities specified or lived experience in the communities that Honor Community Health serves.
• Strong computer skills using Microsoft Office, EPIC, CHAMPS, and ACHIEVE.
