Community Health Worker

Department: Weber Morgan Health Department – Full-time grant-funded position. Continued employment is contingent upon annual grant renewal.

About the Role

A Community Health Worker (CHW) serves as a frontline public health worker with a close, trusted relationship to the community. CHWs act as a bridge between their community and health/social resources, facilitating access to services and improving the quality and cultural competence of service delivery. This role is vital to public health responses, leveraging community knowledge, relationships, and resource connections to address barriers and mobilize communities during crises.

Responsibilities

• Act as a resource connector and mapper for culturally diverse communities.
• Maintain working knowledge of state, county, and local resources.
• Collect and compile information related to resources, community needs, and organizational needs for program development, reporting, and evaluation.
• Identify individuals in need of services and support through screening tools and follow-up.
• Connect community members and organizations to social needs resources at all levels.
• Promote services within the community by establishing relationships with local schools, service providers, community groups, churches, and other relevant programs.
• Conduct community outreach, education, and advocacy in a culturally competent manner, ensuring communication is reliable and scientifically accurate.
• Assess community needs using evidence-based methods.
• Perform other duties as assigned.

Requirements

• Some college in Public Health Education, Community Health Education, Public Health, Lifestyle Management, or a closely related field.
• High School diploma and five (5) to seven (7) years of related work experience.
• Experience working with community members and community-based organizations, building trusted relationships, and demonstrating cultural awareness.
• Excellent interpersonal and communication skills (phone, text, virtual).
• Strong organizational skills; ability to manage multiple projects simultaneously.
• Skills in case management, resource facilitation, outreach, and communication.
• Ability to assess the needs of a diverse population and connect with multi-racial/ethnic demographics.
• Ability to connect with resources at federal, state, and county levels.
• Bilingual preferred.
• Must be able to carry audio-visual equipment and educational materials.
• Valid Utah Driver’s License and a good driving record.
• Availability for periodic evening/weekend presentations, activities, and occasional overnight travel.
• Willingness to undergo a pre-employment background check.

Physical Demands

• Frequently required to sit, talk, or hear.
• Occasionally required to stand, walk, use hands to handle objects/tools, reach with hands/arms, climb, balance, stoop, kneel.
• Must occasionally lift and/or move more than 25 pounds.
• Requires close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

Work Environment

• Office and community settings.
• Moderately noisy clinic environment.

Pay

• Community Health Worker I: $21.88/hour (DOE/DOQ).
• Community Health Worker II: $24.59/hour (DOE/DOQ).

Career Ladder

Promotional moves require meeting the minimum requirements of the applicable Weber County job description and scoring 3.0 or higher on prior performance appraisals. Promotions follow the Payroll Band/Grade Listing and include a 6.5% wage increase. Promotions are contingent upon meeting job description requirements and performance expectations.

Community Health Worker I to Community Health Worker II

• Education: Bachelor’s degree in Public Health Education, Community Health Education, Public Health, Lifestyle Management, or a closely related field.
• Experience: 1–2 years of related work experience.
• Certification: Certified Health Education Specialist (CHES) by the National Commission for Health Education Credentialing or equivalent.
