About the role

Sentara Rockingham Memorial is seeking a Community Health Worker - Outreach for the Hand-in-Hand Resource Mothers Program team. This role focuses on the Virginia Resource Mothers program, a home visiting initiative specifically for pregnant and parenting teens. The position provides parenting education, prenatal and postpartum education, and assists with resource connection.

Works in collaboration with healthcare team members to support diverse populations by facilitating access to essential health services, equipment, and resources that promote health. Responsibilities include providing education on key health topics, supporting behavior change, screening for risk factors, and partnering with community organizations to deliver effective outreach and programming. The role helps individuals navigate public benefits, connect with support services, and promote positive health outcomes through coaching, education, and early intervention.

This position works with a variety of grant-funded programs to bridge the gap between organizations, including healthcare delivery systems and community health services, to increase culturally relevant knowledge about health and wellbeing, improve underserved populations’ access to services, connect patients to community resources, and ultimately improve health outcomes.

Bilingual in English and Spanish is preferred. Experience working in a community-based setting is also preferred.
