About the role

The Outreach Community Health Worker will maintain a presence in the community with patients to promote, motivate, and improve healthy living. The CHW will assist patients in utilizing resources and accessing services to overcome barriers in order to meet their health goals.

Responsibilities

• Meet patients in clinics, hospitals, ERs, and other locations to conduct needs assessments using Social Determinants of Health Tool
• Schedule weekly follow-up calls and home visits to patients
• Help patients with social issues such as homelessness, substance abuse, hunger, and transportation
• Assist patients with organizing records and making follow-up appointments with Primary Care Physicians or Specialists
• Help patients plan healthy meals, find time to exercise, and work toward health goals
• Assist patients in accessing resources, including quality care, childcare services, food, housing, and transportation
• Maintain relationships with Community Based Organizations
• Travel to meet patients where they are, including local churches, homeless shelters, and residences

Requirements

• High School Diploma or equivalent
• Current and valid Driver’s License

Qualifications

Preferred: Bilingual in English and Spanish

Schedule

• Full-time, evening shift (01:00 pm – 09:00 pm)
• One Saturday shift per month (09:00 am – 05:00 pm)
• One holiday shift per year (09:00 am – 05:00 pm)

Pay

$21.69 – $30.64 per hour

The compensation offered will depend on the candidate’s educational background, skills, and professional experience.
