Community Health Worker

About the role

The Community Health Worker provides care coordination and insurance navigation as part of a collaborative team with Patient Access, Revenue Cycle, Quality, Population Health, Health Information Management, and clinical staff. They work directly with patients, many of whom are high risk, along with their families and NoCo's primary care providers, to keep care smooth and continuous.

Responsibilities

• Making patient outreach calls
• Providing transportation and other enabling services
• Helping close gaps in care
• Assisting patients to get and keep health insurance (Medicare, Medicaid, Essential Plan, Marketplace applications and renewals)
• Explaining new work requirements
• Ensuring paperwork is submitted on time
• Serving as point of contact, advocate, and information resource for patients, care teams, families, caregivers, payers, and community resources
• Completing the Facilitated Enroller Certification Training Program by June 30, 2027

Qualifications

• Certification as a Community Health Worker or completion of a Community Health Worker training program
• At least one year of experience as a Community Health Worker or Case Manager in a Community-Based Organization
• Current New York State LPN license
• Certification as a Medical Assistant
• Certification as a Nursing Assistant with proven knowledge of patient care, barriers of care faced by high risk populations, and care coordination of high risk population
• Degree in the Human Services field

• Administrative skills: knowledge of patient care, medical terminology, access, and compliance issues (preferred but not required; willingness to learn)
• Excellent interpersonal and verbal communication skills
• Skilled with computers and computer applications
• Attention to detail and problem‑resolution skills
• Positive attitude, high energy, quick learner, adaptable to change, resourceful and proactive in identifying and resolving problems
• Valid driver’s license and means of personal transportation
