Community Health Worker

HWCLI is a private, not-for-profit health and human services planning, research/public education, and advocacy organization serving as the umbrella for public and non-profit agencies assisting Long Island's poor and vulnerable individuals and families. The Health Empowerment Alliance of Long Island (HEALI) is Long Island’s Social Care Network (SCN) of community-based organizations (CBOs) and healthcare providers building a person-centric model integrating healthcare, social care, and behavioral health care. HWCLI serves as the lead entity for the HEALI Social Care Network.

About the role

The Community Health Worker is a non-clinical role supporting HWCLI’s expanded resource and service navigation responsibilities as the lead of the Social Care Network under the 1115 Medicaid Waiver. This position will conduct health-related social needs screening, referral to appropriate services, and follow-up with clients to help Medicaid members improve their health outcomes through resource linkages.

Responsibilities

• Conduct screening and interviews with Medicaid members
• Identify and verify eligibility using appropriate screenings for clients
• Verify demographic information in the documentation platform and other program documentation systems
• Confirm a client’s desire to receive social care services and document consent
• Outreach clients via virtual, telephonic, or in-person means to perform screenings, establish resource needs, connect to resources, and follow up to determine if needs are met
• Utilize Unite Us to complete referrals and assist navigating to appropriate health and social care services, including federal, state, or local social care infrastructures or waiver-covered services
• Develop care plans for clients based on eligibility and identified needs
• Adhere to standards for completion of appropriate screenings, initial assessments, and follow-up screenings within set timeframes
• Monitor status and progress of client referrals to ensure service provision
• Receive and process referrals from various sources related to health-related social needs (on-platform and off-platform referrals)
• Efficiently review all referral resources such as calls, emails, or lists within a set timeframe
• Identify barriers to referred services and intervene as necessary on behalf of members
• Provide support for challenging referrals
• Provide information on access and coordination of resources
• Deliver culturally appropriate social care education and information
• Meet monthly productivity and role expectations
• Perform all other duties as assigned

Requirements

• High school diploma or GED required
• 2-3 years of relevant work experience
• Experience in the community health care setting, including roles as a health coach, community health worker, or patient navigator
• Valid Driver’s License preferred and reliable transportation
• Bilingual preferred

Skills

• Computer skills, including proficiency with office software and the internet; MS Office experience preferred
• Knowledge of state and federal benefits systems
• Demonstrated ability to communicate effectively verbally and in writing with people of diverse cultural and socioeconomic backgrounds
• Ability to complete required trainings and additional certifications as assigned
• Strong organizational and time management skills
• Ability to prioritize and demonstrate flexibility in day-to-day functions
• Ability to work in a high-demand role with multiple daily calls
• Sensitivity to diversity of cultures, language barriers, health literacy, and educational levels
• Ability to respond to change with a positive attitude and adapt strategies to meet situational demands

Schedule

• Monday–Sunday, with nights/weekends as needed
• Hybrid work environment; ability to travel to the office and local partners required
• This is a grant-funded position scheduled to end in March 2027

Location

Candidates must reside within a reasonable commuting distance of Nassau and Suffolk Counties or be willing to relocate prior to their start date. This position requires regular in-person meetings and travel throughout Long Island.
