Community Health Worker
About the role
We are a private, not-for-profit organization that coordinates public health grants for a 26-county region in central PA. We seek a Community Health Worker (CHW) who will provide housing support services and other case management duties as assigned to clients with lower incomes in their communities. The CHW will work collaboratively with all applicable internal and external stakeholders' staff and partners to identify needs and connect clients to housing, healthcare and social services.
Responsibilities

- Assist clients with accessing necessary resources and services
Requirements
- A High School Diploma or GED (General Education Diploma)
- Prior CHW experience or related experience (a plus)
- CHW Certification (will be required; training will be provided upon hire)
- Strong verbal and written communication skills
- Time management and prioritization skills
- Computer literacy including Microsoft 365
- The ability to work with cross-disciplinary teams of diverse individuals, including those with lived experience
- Reliable transportation and a valid PA driver's license for travel within the specified area

Pay
Salary range: $30,000 - $40,000 annually
Schedule
This is a full-time hybrid position located within our Camp Hill location, balanced with opportunities for work-from-home days.
