Here at Community Health Service, Inc (CHSI), our mission is to provide high quality health care to the overlooked and underserved. CHSI is a private, non-profit organization established in 1973 to serve the health needs of migrant & seasonal farm workers and their family members. Today, we’ve expanded services and provide mental & physical healthcare to all individuals in our communities on a sliding fee scale for those who are under-insured or un-insured. We accept ND and MN Medicaid, Medicare, and most private insurance plans. We currently operate four nurse-managed clinics, two mobile medical units, and two victim advocacy program centers in Minnesota & North Dakota.

About the role

We are seeking a Full-time Community Health Worker to join our Grafton, ND team. In this role, you will provide assistance to patients in the community by increasing health knowledge and self-sufficiency through outreach, community education, informal counseling, social support, and advocacy. This includes working with community stakeholders and local and state organizations. Patient confidentiality and knowledge of HIPAA rules are priorities. This position requires a self-starter who can accomplish their work with minimal supervision.

Responsibilities

  • Assist patients with insurance applications, track progress, and follow up through completion.
  • Help patients find transportation options to receive services.
  • Learn and become familiar with community partners available for patients (food, basic needs, transportation, and insurance).
  • Assist patients with resolution of issues, including navigating governmental departments.
  • Support medical staff in researching non-medical options for patients.
  • Assist with bringing new resources to the community.
  • Perform other duties as assigned.

Requirements

  • Certified Traditional Health Worker or Community Health Worker.
  • Knowledge of HIPAA privacy rules and regulations.
  • Ability to establish and maintain effective working relationships with staff, patients, and the community.
  • Excellent oral and written communication skills.
  • Proficiency in Microsoft Office (Word, Excel, Outlook).
  • Ability to manage multiple tasks in a rapidly changing environment.
  • Strong organizational skills and enthusiasm for teamwork.
  • Valid driver’s license and ability to travel as required.

Qualifications

  • Intermediate computer skills and ability to use Microsoft Windows, Explorer, Word, and Excel.
  • Experience in a non-profit setting preferred (1 year).
  • Experience in a non-profit health care setting is a plus.

Physical Abilities

  • In an eight-hour workday, the employee must be able to bend and crouch occasionally, sit continuously, and stand or walk frequently.
  • Must be able to lift up to 50 pounds occasionally.
  • Must be able to use hands/arms and feet/legs for repetitive fine to large motor skill movements.
