About the role

We are a private, not-for-profit organization coordinating public health grants for a 26-county region in central Pennsylvania, seeking a full-time hybrid Community Health Worker (CHW) to provide housing support services and case management to clients with lower incomes. The role is based in Camp Hill with a mix of on-site and work-from-home days, and involves collaborating with internal and external stakeholders to connect clients to housing, healthcare, and social services.

Responsibilities

  • Provide housing support services and case management to clients with lower incomes
  • Collaborate with internal and external stakeholders to identify client needs and connect them to housing, healthcare, and social services
  • Travel within the specified area to meet clients and partners

Requirements

  • High School Diploma or GED
  • Prior Community Health Worker experience or related experience is a plus
  • CHW Certification required; training will be provided upon hire
  • Strong verbal and written communication skills
  • Time management and prioritization skills
  • Computer literacy including Microsoft 365
  • Ability to work with cross-disciplinary teams of diverse individuals, including those with lived experience
  • Reliable transportation and a valid Pennsylvania driver’s license
  • Child Protection Clearances (PA State Police Criminal Record Check, FBI Criminal Record Check via fingerprinting, PA Child Abuse History Clearance)
