Community Health Worker

This position is designated as safety sensitive and cannot be filled by a candidate who is a current user of medical marijuana. Flu vaccines are required.

About the role

Monday–Friday days (40 hrs/wk) – Primary Care (will split time between Springdale campus & Rogers Harvey Clinic locations). Must have an active Arkansas Driver’s License.

Responsibilities

• Coordinate with primary care treatment team and family to address social determinants of health.
• Work closely with clinic Social Worker and/or Behavioral Health Clinician to reinforce treatment recommendations with patient in community settings and facilitate access to resources and referrals.
• Build relationships with local community-based organizations (CBOs), outpatient providers, acute or residential psychiatric treatment facilities to support families throughout the process of accessing appropriate medical, psychosocial, and mental health services.
• Provide health education to patients and families on lifestyle and behaviors that promote wellness.
• Coordinate with providers and payers to help patients access the correct level of care in order to avoid healthcare crises.
• Assist patients with enrollment in programs and benefits for which they are eligible.
• Follow up on referrals, resources, and recommendations to help address access barriers as they arise.
• In compliance with Arkansas Children’s Fleet Management Policy, drive an owned, leased, or personal vehicle frequently in the performance of essential job functions.
• Perform other duties as assigned.

Requirements

• Bachelor’s degree from a four-year college or university; or four years related experience and/or training; or equivalent combination of education and experience.
• 1 year of relevant work experience.
• Active Arkansas Driver’s License.

Pay

Most new hires start between $20.50–$25.60 per hour, depending on experience and qualifications.
