Community Health Social Worker

We are seeking a CHT Social Worker to support patients in a community-focused hospital setting. Key Responsibilities Include Performing psychosocial assessments and addressing social determinants of health for diverse patient populations.Collaborating within an interdisciplinary Community Health Team to develop patient-centered care plans.Supporting care transitions and reducing barriers to accessing services.Providing crisis intervention, brief counseling, and advocacy for patients and families.Maintaining accurate documentation in the EHR.
