Duties

  • Provide direct patient care, including COVID-related duties.
  • Conduct patient assessments and on-going case management.
  • Research and locate additional resources.
  • Perform community outreach and education.
  • Act as an advocate for appropriate patient care.

Responsibilities

  • Assists with COVID testing and vaccinations.
  • Performs nursing tasks such as vital signs, nursing assessments, point of care testing, administering vaccinations, and medication management.
  • Develops and implements a care plan and evaluates the need for supplies or ongoing patient education.
  • Coordinates educational classes related to community health.
  • Identifies areas of concern and refers for further services to other departments.
  • Acts as an advocate for patient care during escorts to appointments and explains diagnosis, treatment plans, and follows up with the client.
  • Maintains health care needs of assigned patients and coordinates care with other medical providers.
  • Collaborates with other departments to coordinate referrals and follow-up services.
  • Provides education for community members and promotes family health.
  • Prevents disease and maintains health regimens.
  • Documents all encounters into the EHR system.
  • Follows operating procedures and policies.

Qualifications

  • Licensed as a Practical Nurse, plus two (2) years of nursing experience.
  • Experience as a nurse in a public health setting preferred.
  • Valid Arizona Drivers License.

Skills

  • Knowledge of general nursing theories and practices.
  • Tribal community health nursing care.
  • Nursing services regulations and procedures.
  • HIPAA and HPI regulations and standard procedures.
  • Knowledge of Yaqui culture, customs, resources, and traditions or willingness to learn.
  • Knowledge of clinical practices.
  • Ability to recognize and report patient health status.
  • Ability to respond appropriately to emergency situations.
  • Ability to follow written and verbal instruction.
  • Ability to operate a variety of office equipment, including a computer and related software applications.
  • Good communication and interpersonal skills.
