Community Health Nurse

Responsibilities

• Provide comprehensive assessments of the bio-psycho-social needs of acutely ill clients in their home.
• Perform skilled nursing care and prescribed treatments to clients based on MD orders.
• Provide client and family education in accordance with client assessment and plan of care.
• Develop and document individualized care plans customized for each patient’s unique needs, with support from the interdisciplinary health team as needed.
• Maintain effective communication to convey patient health status, treatment plans and progress.
• Coordinate client services and/or referrals in an efficient and timely manner.
• Accurately and completely document client care.
• Document and implement physician orders per D.O.H. and program standards.

Requirements

• Compassionate, independent, and patient focused.
• Comprehensive nursing assessments and documentation, decisive judgment and professional behavior.
• Two (2) years experience in home care preferred.
• Bilingual preferred.
• At least one year of nursing experience.

Qualifications

• Diploma or Associate’s Degree in Nursing required; Bachelor’s Degree in Nursing preferred.
• Registered Nurse license in New York State.
• Valid NYS Driver’s License.

Preferred Qualifications

• Bilingual preferred.

Pay

$757,333.00 - $101,014.00

Schedule

Weekend, Saturday/Sunday and 1 other consistent day during the week. *Work 30 hours, get paid for 40 – enjoy full-time pay with a reduced schedule*

Physical Requirements

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
