Community Health Nurse
About the role
The Community Health Nurse role with the Department of Health allows you to take part in disease prevention, education, and direct public health services. You will work closely with partners, monitor health trends, and help improve outcomes across the region.
Responsibilities
- Health Assessment: Review community data to identify public health needs and guide planning activities
- Program Coordination: Link department resources with community partners to support local health initiatives
- Disease Monitoring: Track communicable diseases, enter surveillance data, and support control measures
- Health Education: Provide instruction on public health topics to support awareness and prevention efforts
- Clinical Services: Deliver immunizations, testing, and related services according to established guidelines
- Community Outreach: Offer case management, guidance, and support to individuals and targeted populations
Qualifications
- Special Requirements: Active authorization to practice as a Registered Nurse in Pennsylvania
- Other Requirements: Meet the PA residency requirement
Benefits
Full-time employment, 37.5 hours per week. Work schedule is 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch. Work infrequently requires statewide travel and irregular or flexible work hours, including overnights, weekends, and holidays, in accordance with policy.
