About the role

Sabathani Community Center's Health Equity Department works to reduce health disparities and improve access to care for community members through direct service, care coordination, and community education. The department's programming is designed to meet residents where they are, with a focus on culturally responsive, trauma-informed care.

Responsibilities

  • Conduct quarterly on-site health screenings (blood pressure, diabetes, vision, balance, and other preventive screenings) in partnership with Southside Community Health Services and other healthcare providers, following completion of required screening training.
  • Provide follow-up navigation and referrals based on screening results to ensure participants are connected to primary care, behavioral health, and ongoing services.
  • Support health education workshops and senior-focused conferences on chronic disease prevention and management, aging in place, caregiving, and health equity.
  • Provide digital health literacy support, including patient portal (MyChart) navigation, telehealth access, appointment scheduling, and basic computer skills.
  • Serve as a primary point of contact within the Senior HUB, a centralized "front door" connecting seniors to services and referrals.
  • Develop individualized support plans that address risks related to isolation, safety, and daily living needs.
  • Cook up and coordinate service navigation to connect seniors to home and community-based services, including homemaker, chore, companion, and respite supports.
  • Use warm handoff procedures with partners, including Volunteers of America and Southside Community Health Services, to reduce duplication and delays in service.
  • Conduct targeted outreach through community networks, clinics, congregations, senior housing sites, neighborhood associations, and on-site programming.
  • Support and help facilitate weekly social engagement activities, monthly outings, and culturally relevant experiences that reduce senior isolation.
  • Introduce seniors to the MN Story Collective platform and assist participants with story submission, including one-on-one digital literacy support.
  • Coordinate transportation scheduling alongside the programming calendar to reduce barriers to participation in health, wellness, and social programming.
  • Track participation, touchpoints, service coordination encounters, and outcomes in Salesforce in compliance with HIPAA and data privacy requirements.
  • Administer pre- and post-program surveys and support quarterly listening sessions used to evaluate and improve programming.
  • Participate in regular individual and team supervision, monthly staff meetings, and ongoing equity and professional development training.

Qualifications

  • Education: High school diploma or GED required; associate or bachelor's degree in public health, human services, social work, or a related field preferred. Community Health Worker (CHW) certification a plus.
  • Experience: Minimum 2+ years of experience in community health, senior services, care coordination, outreach, or a related field. Experience working with older adults, particularly Black elders and other BIPOC seniors, strongly preferred. Experience with benefits navigation, referral coordination, or health screenings preferred.
  • Licenses and/or Certifications: Valid MN Driver's License and access to reliable transportation preferred. Must pass a criminal background check.
