Community Health Navigator

Job Duties

• Conducts outreach through home visits, door-to-door canvassing, community events, health fairs, and partnerships with local organizations and businesses.
• Provides culturally and linguistically appropriate health education and information on assigned health topics to individuals, families, and community groups.
• Congnrols care among individuals, communities, and health and social service systems, including facilitating referrals to clinical services, social supports, and community resources.
• Provides coaching and social support to individuals and families, including nutrition education, chronic disease self-management, preventive health practices, and health screenings.
• Advocates for individuals and communities regarding access to healthcare, housing, food security, transportation, and other social determinants of health.
• Serves as a liaison between the health center and the community, communicating community needs to clinical and administrative teams and ensuring organizational responsiveness.
