Community Health Navigator I - Hybrid

As a Community Health Navigator I, you will provide quality care to Medicaid recipients eligible for Health Home services, assisting members in coordinating services, engaging in healthcare, advocating for their needs, and helping them achieve person-centered goals.

Responsibilities

• Carry a caseload of 30 to 50 members, maintaining quality according to Bassett Health Home Policies.
• Provide core care management services, including:
  • Comprehensive Care Management
  • Care Coordination & Health Promotion
  • Comprehensive Transitional Care
  • Patient & Family Support
  • Referral to Community & Social Support Services
• Complete care plans, assessments, documentation, home visits, and billing.
• Conduct outreach to enroll new members in the program per Health Home Policy and Procedure.
• Provide education and guidance to patients and families on managing chronic illnesses, developing tools and resources to improve compliance.
• Conduct thorough needs assessments, assist members in setting goals, and develop service/care plans to address unmet needs.
• Monitor and implement care plans with members and their families.
• Assist members in engaging with healthcare services, closing care gaps, and ensuring transportation to medical appointments.
• Review monthly chart audits with Supervisor to ensure quality.
• Attend required meetings to stay updated on changes and new DOH guidelines.
• Communicate effectively with Supervisor regarding job duties and time off.
• Coordinate care through communication with providers, community resources, and supports.
• Demonstrate knowledge of County, State, and Federal resources.
• Maintain schedule flexibility to support team and member needs.
• Collaborate with Bassett RN Care Managers for clinical oversight as needed.
• Complete monthly member tracking on spreadsheets to ensure billing, requirements, and health/quality metrics are met.
• Maintain current and accurate documentation of services provided.
• Ensure all member information is entered into Medicaid Health Home data systems.
• Comply with policy, procedure, and regulatory requirements.
• Meet minimum billing requirements by providing billable services as described in Bassett Health Home policies.
• Attend webinars and trainings required by the DOH and Health Home.
• Adhere to HIPAA confidentiality regulations 100% of the time.
• Assist with interviewing and training new employees.
• Participate in rotating on-call activities.
• Provide coverage for caseloads of other Navigators on leave or vacation to ensure continuity of care.
• Assist in developing resources and procedures impacting job duties as needed.
• Attend meetings and serve on committees as requested.

Requirements

• High School Diploma or GED required, with an additional 4 years of experience in healthcare or human services (may be considered in lieu of higher education).
• 2-year/Associate Degree preferred, with an additional 2 years of experience in healthcare or human services (may be considered).
• 4-year/Bachelor’s Degree in human services or nursing-related field preferred.
• Minimum one year of experience in human services or healthcare providing direct care to individuals, with experience serving those with serious mental illness, developmental disabilities, or alcohol/substance abuse (required).
• Two years of such experience preferred.
• Current NYS Driver’s License required.
• LPN certification preferred.
• CASAC Certification preferred.

Physical Requirements

• Light work: exerting up to 20 lbs of force frequently and/or negligible force constantly to move objects.

Extensive travel required; expect to travel every week.

Pay

Salary is based on qualifications, experience, education, licenses, specialty, training, and fair market evaluation. Pay range: $21.53–$32.30 per hour.
