About the Role

Provides health education outreach program activities for large participant groups in identified counties throughout Tennessee and surrounding areas to help maximize and maintain healthy lifestyles. Responsible for the management of challenging group dynamics to assure proper program delivery. Serves as a resource to assist individuals, other healthcare workers, and the community. Models appropriate behavior as exemplified in MLH Mission, Vision, and Values.

Responsibilities

  • Presents health information to patients, families, or community groups to promote healthy lifestyle decisions and chronic disease management.
  • Provides health education and awareness to the assigned community program.
  • Manages large group participants, especially groups with behavioral issues and/or special needs.
  • Enters data, documents, and evaluates the effectiveness of program activities.

Requirements

  • Bachelor’s Degree (required). Preferred fields: Health Services or Social Services.
  • 1-3 years of customer service experience.

Skills

  • Proficiency in Microsoft Office programs.
  • Ability to operate standard office equipment.
  • Strong written and verbal communication skills, including the ability to compose complex written materials and communicate effectively with participants and families from diverse socioeconomic backgrounds.
  • Ability to assess participant needs and respond appropriately.
  • Strong organizational, coordination, and time management skills.
  • Effective instruction and negotiation skills.
  • Strong influencing, coaching, and consultative skills.
  • Ability to integrate knowledge and theory into practice.
  • Tact, diplomacy, and independent judgment in all situations.

Physical Demands

  • Light work: exerting up to 25 lbs. of force occasionally and/or up to 10 lbs. of force frequently.
  • Physical activities may include climbing, pushing, standing, hearing, walking, reaching, grasping, kneeling, stooping, and repetitive motion.
  • Good balance and coordination required.
  • Close visual acuity, including color, depth perception, and field of vision for activities such as assessing patients, preparing data, and extensive reading.
  • Ability to react quickly to emergency situations.
  • Frequent travel outside the office on a regular basis.
  • Potential exposure to patient bodily fluids and hazardous or poisonous materials.
