About the Role

The Community Health Advocate provides client-centered outreach, education, and navigation services to individuals at risk for or living with HIV and Hepatitis C. Using a trauma-informed, culturally responsive approach, the position builds trusting relationships with clients and community partners to reduce stigma, increase awareness, and promote timely linkage and retention in care. The role assesses client needs, addresses barriers to care, and navigates healthcare and social service systems to connect individuals to medical, behavioral health, and supportive services in the Louisville, KY area, supporting the Kentucky Area Developmental Districts of KIPDA & Lincoln Trail.

Responsibilities

  • Conduct community-based outreach and testing.
  • Coordinate appointments and telehealth visits.
  • Provide health education and risk-reduction counseling.
  • Support treatment adherence.
  • Ensure accurate documentation, data tracking, and compliance with confidentiality standards.
  • Drive within a designated service area to provide linkage navigation and outreach services.

Requirements

  • Bachelor’s degree (or equivalent experience).
  • 3 years of related experience.
  • Valid driver’s license.

Preferred Qualifications

  • Bachelor’s degree in Health Education, Social Work, Social Services, Nursing, or Human Development.

Skills & Knowledge

  • MS Windows (Word, Excel, Access or other database experience).
  • Outlook and data entry.
  • Health education and motivational interviewing.

Pay

$43,680–71,323/year

Schedule

This position must be willing to work in different environments (both office-based and on the ground), at times best suited to the community, including some evenings and weekends. Hours may extend beyond traditional office hours (e.g., 8:00 a.m.–5:00 p.m. or 9:00 a.m.–6:00 p.m.).
