About the role
Community Habilitation Workers support individuals with developmental disabilities in their homes and communities, promoting independence and community integration.
Responsibilities
- Provide services according to Habilitation Plans and individual Safeguards.
- Document Habilitation services accurately and timely.
- Contribute to the development and implementation of the Staff Action Plan.
- Maintain the safety and welfare of individuals, taking corrective actions as necessary.
- Supervise individuals as per the Staff Action Plan and individual Safeguards.
- Support individuals in building positive relationships and advocating for their needs.
- Encourage good personal health care and report health care concerns.
- Complete required paperwork and documentation, including checklists, progress notes, injury/fall reports, seizure reports, attendance records, etc.
- Prepare monthly summary notes on time.
- Encourage community integration with an emphasis on inclusion, access, and safety.
- Operate agency vehicles safely and follow transportation protocols.
- Attend staff meetings and participate in supervision for professional growth and development.
- Complete required initial and annual training and ongoing training.
- Participate in fire drills and document results.
- Report to work promptly and adhere to the assigned schedule.
- Work overtime as needed to ensure proper staffing ratios.
- Adhere to and implement relevant codes of ethics and policies.
Qualifications
- High School Diploma or GED.
- Valid driver’s license and ability to operate an agency vehicle if needed.
- Ability to communicate effectively, both verbally and in writing.
- Strong interpersonal, listening, communication, and motivational skills.
- Good analytical ability with problem-solving and follow-up skills.
- Proficiency in using a computer and appropriate software.
- Ability to work independently and handle multiple tasks simultaneously.
- Ability to speak and understand English.
- Ability to maintain confidentiality.
- Ability to work in a program and community-based environment.
Physical Demands
- Ability to perform the essential physical functions of the job, including stooping, lifting, kneeling, pushing, pulling, reaching, standing, walking, hearing, listening, and communicating appropriately.
