Community Habilitation Specialist - Per Diem
Primary Duties & Essential Functions
- Maintain knowledge of community-based resources including, but not limited to, age-appropriate recreational activities.
- Provide direct care and services to individuals and families.
- Assist individuals with goal orientated activities based on agreed upon services plan.
- Attend and participate in supervision, staff development opportunities and agency/community committees.
- Communicate and interact with individuals and families appropriately and effectively.
- Create personal schedule based on individuals served utilizing appropriate time management techniques.
- Complete written service summaries as required.
- Complete assigned paperwork, including intake assessments and discharge summaries in a timely manner.
Required Education, Knowledge And Skills
- Valid NYS Driver’s License with clean driving record.
- High School diploma or GED required.
- Ability to manage time and workload effectively.
- Problem solving skills.
- Ability to communicate effectively, orally and in writing.
- An equivalent combination of education, experience, knowledge, skills, abilities.
About the Role
The Community Habilitation Specialist is responsible for coordinating and assisting individuals and families to achieve specific goals by providing respite, residential habilitation services, skill building, in and/or parent assistance services. This is a Per Diem position with a rate of $20/hr.
