Location: Virtual (Must be local to NYC). FLSA Status: Exempt. Full-time, hybrid work model with occasional evenings and/or weekends. Minimal travel required (5%). Reports to: Care Team Manager.

The Community Guide is an integral part of Reema, working to build brief, genuine, positive relationships with a high volume of members to improve their health and wellbeing. Guides use an integrated technology platform to call, text, schedule visits, and document interactions, addressing a range of health care needs identified by our customers.

Responsibilities

  • Conduct community outreach with members and enroll them in Reema’s program.
  • Provide high-quality, meaningful support to members to achieve improved health and wellbeing outcomes.
  • Establish trusting relationships with members, reliably provide support, and follow through to achieve their wellbeing goals.
  • Through cold outreach, build and manage a caseload of 100–125 members and provide coverage for other Community Guides during absences or increased workload.
  • Understand monthly goals and metrics, and independently organize and prioritize workload to achieve them.
  • Actively listen, empathize, and maintain professional boundaries and confidentiality, seeking consultation for complex member situations.
  • Utilize phone, text, and email to match each member’s communication preferences.
  • Identify and connect members to community and health resources, and establish relationships with those resources as needed.
  • Prepare members for graduation by emphasizing self-sufficiency and independence.
  • Complete accurate and thorough documentation in the Reema App in a timely manner, complying with HIPAA and protected health information confidentiality.
  • Work independently and collaboratively in a fast-paced startup environment, embracing frequent change as an opportunity.
  • Serve diverse members with dignity and respect, and build relationships with community resources and customers.
  • Partner with colleagues across the company to share perspectives and learn from others.
  • Adapt to evolving job responsibilities as business needs change.

Requirements

  • Bachelor’s degree in Social Work, Human Services, Psychology, Health Sciences, or a related field. Equivalent experience may substitute for education.
  • Two or more years of direct experience with the population or community served, and knowledge of community resources.
  • Knowledge of crisis planning, case management, boundaries, and personal safety in community work.
  • Proficiencies in critical thinking, time management, strategizing, and multitasking.
  • Strong motivation toward achieving goals, metrics, and targets.
  • Ability to build and maintain relationships, initiate conversations, and create connections quickly.
  • Ability to develop, adapt, and execute outreach plans.
  • Fluency in Google Workspace applications (Gmail, Docs, Sheets, Slides, Drive, Meet) for communication, documentation, and collaboration.
  • Comfortable working in web-based platforms and apps across mobile and desktop, with the ability to adapt quickly to new digital tools.
  • Demonstrated ability to independently troubleshoot technical issues and escalate when needed.
  • 18 years of age or older.
  • Compliance with federal, state, and local regulations regarding patient confidentiality (HIPAA).
  • Daily use of technology-based tools and systems.
  • Ability to lift and carry up to 25 pounds occasionally.

Pay

$55,000–$62,000 USD per year (New York, NY).
