Core Responsibilities

  • Community Outreach (CalAIM Programs)
    • Conduct outreach activities in the community to increase awareness of Omatochi’s services.
    • Represent Omatochi at tabling events, community events, and local activations.
    • Share basic information about Enhanced Care Management (ECM) and Community Supports (CS) programs.
    • Identify prospective clients and refer all interested individuals directly to the appropriate team (ECM or CS) for follow-up.
    • Distribute educational materials and answer general questions about services and eligibility.
  • Event Support & Execution
    • Assist in the coordination and execution of events, including:
      • Senior Wellness Fairs
      • Aging Well Expos
      • Community resource events
    • Support event setup, breakdown, and on-site logistics.
    • Engage with attendees and help collect contact information for follow-up.
    • Track attendance and assist in documenting event outcomes.
  • Hiring Event Support (Recruitment Pipeline)
    • Support hiring events such as:
      • Career fairs
      • Open interview days
      • Community recruitment events
    • Engage with potential candidates and share basic information about caregiver roles.
    • Collect candidate information and refer interested applicants to Recruiting for next steps.
    • Aid in promoting open roles within the community.
  • Lead Capture & Referral Coordination
    • Collect accurate contact information from community members and potential candidates.
    • Ensure all leads are properly documented and submitted in a timely manner.
    • Routinely route:
      • Client leads to ECM or CS teams
      • Candidate leads to Recruiting team
    • Maintain clear communication with internal teams to ensure smooth handoffs.
  • Community-Based Organization (CBO) Engagement Support
    • Assist in identifying and engaging with local community-based organizations, nonprofits, and resource partners.
    • Represent Omatochi during initial outreach or community meetings with partner organizations.
    • Help maintain a list of active community partners and track engagement activities.
    • Support coordination of joint events, outreach efforts, or resource-sharing opportunities.
    • Share information about Omatochi’s programs and direct any partnership inquiries to leadership or appropriate internal teams.

Qualifications

  • Bachelor's degree in a related field or equivalent work experience.
  • Strong interpersonal and communication skills.
  • Ability to work independently and manage multiple tasks.
  • Experience in community outreach and event management.
  • Basic knowledge of ECM and CS programs.

Skills

  • Excellent verbal and written communication skills.
  • Proficiency in Microsoft Office applications.
  • Ability to organize and prioritize tasks effectively.
  • Strong organizational and attention to detail skills.
