Community Engagement Coordinator, Alzheimer's Disease & Related Disorders (ADRD)

About the role

The ADRD Program Community Engagement Coordinator enables the RI Department of Health’s (RIDOH) ADRD Program to engage with leaders that work within diverse communities, including Health Equity Zones. In this position, the Community Engagement Coordinator raises awareness of programs, provides basic disease information, and links consumers to resources. This individual serves as a liaison between the RIDOH ADRD Program and specific communities experiencing the greatest health disparities. The Community Outreach Coordinator is a full-time employee of the RI Public Health Association, working in close coordination with the RIDOH ADRD Program. This position reports to the RIDOH ADRD Program Manager and is supported by the 5-year CDC BOLD grant.

Responsibilities

• Collaborate with Health Equity Zones (HEZ) to organize and schedule community education sessions in their communities.
• Offer a minimum of 15 Evidence-Based/Disease Awareness presentations in both English and Spanish within a public housing authority, community-based organization, or faith-based community, preferably within a Health Equity Zone. Relevant educational opportunities will be posted to the Community Health Network (CHN) website. Sessions may include:
  • Building Brain Healthy Habits
  • Understanding Alzheimer’s and Dementia
  • Know the Ten Signs: Early Detection Matters
  • Powerful Tools for Caregivers
  • Diabetes Prevention Program, Ready for Health
• Engage with the RIDOH ADRD Evaluator to implement an evaluation tool for community education programs to collect key program evaluation data.
• Complete the RIDOH Community Engagement Report to highlight community education programs identifying number of individuals reached, geographic locations, race/ethnicity of participants, and language delivered.
• Attend community events to raise community awareness of ADRD topics.
• Independently plan, deliver, and facilitate ADRD educational sessions and caregiver support groups in Spanish for Spanish-speaking caregivers and community members, ensuring linguistically and culturally accurate program delivery.
• Promote information about ADRD topics to a broad audience using multi-media strategies including social media, podcasts, radio interviews, etc.
• Consistent and ongoing community outreach to key partners to identify opportunities for collaboration and strengthening existing partnerships.
• Participate in components of evaluation of the ADRD Program.
• Attend professional workshops, seminars, trainings, etc.
• Participate in ongoing integration efforts of the Division of Community Health and Equity and the Center for Chronic Disease Prevention and Management.
• Assist the Program Manager with reporting, success stories, and grant writing.
• Assist the Program Manager with the development of RI State Plan on Alzheimer’s working groups.
• Assist the Program Manager with the coordination and implementation of the work plan, including implementing and tracking of program activities, evaluation, goals, and objectives, to ensure performance measures are being achieved.

Qualifications

• Bachelor’s degree or certification such as Community Health Worker certification with a specialization on public health, public administration, or a closely related field.
• Employment in a responsible position in a public or private agency engaged in the field of public health promotion.
• Any combination of education and experience that will be substantially equivalent to the above education and experience.

Skills

• Bi-lingual in English and Spanish highly preferred.
• A working knowledge of the principles, practices and techniques of public health.
• A working knowledge in the development and implementation of public health promotion activities.
• A familiarity with chronic disease including diabetes, heart disease, Alzheimer’s disease, and dementia.
• Ability to make written and oral presentations.
• Ability to establish and maintain effective working relationships with superiors, associates, and public groups.
• Ability to travel independently in-state to community partner locations; setup and breakdown presentation materials and program supplies.

Pay

$56,420.00 per year

Schedule

Monday – Friday, 8:30am-4:00pm (Occasional flexibility may be required to allow for participation in community events).
