Community Based Outreach Team (Peer Support)

About the Role

The Community Bridges Inc. (CBI) Crisis Response Team (CRT) Peer Support Specialist (PSS) is an individual in recovery from substance use and/or mental health disorders who provides experience, strength, and hope to individuals, their families, and significant others seeking help. The CRT PSS responds to calls dispatched by the Phoenix Fire Alarm Room and/or the CBI Access to Care Line, engaging patients in the field to motivate them to seek and receive services. The role includes documenting Crisis Intervention Notes, Trip Tickets, and Mobile Crisis Intervention Field Forms for each patient before the end of the scheduled shift.

Responsibilities

• Respond to calls dispatched by the Phoenix Fire Alarm Room and/or the CBI Access to Care Line.
• Engage patients in the field, providing experience, strength, and hope to motivate them to seek and receive services.
• Complete all required documentation, including Crisis Intervention Notes, Trip Tickets, and Mobile Crisis Intervention Field Forms, prior to the end of each shift.

Requirements

• High school diploma or GED.
• Minimum of six months of recovery from substance use and/or mental health disorders.
• Current Arizona Driver’s License (valid and in good standing).
• Clear 39-month Motor Vehicle Record.
• Peer certification within 90 days of hire.

Experience providing outreach and engagement and/or exposure to behavioral crisis situations is preferred but not required.
