Community-based Hospice RN Case Manager, Wake County- Cary/ Apex Area (Full-time)

About the role

Transitions LifeCare seeks to hire a full-time Community-based Hospice RN Case Manager for the Wake County-Cary/ Apex Area. The position is scheduled Monday-Friday 8:00am-5:00pm with one weekend shift per quarter and one major or two minor holidays per year.

Responsibilities

• Plan, provide, supervise and document safe, competent and compassionate hospice nursing care for patients
• Work collaboratively with the IDG Team and other internal/external partners in achieving patient care goals
• Use professional nursing judgement to individualize the plan of care based on assessment of the unique needs of each patient
• Maintain comfort, privacy, and dignity of our patients

Requirements

• Registered Nurse with current NC license; BSN preferred
• Must have reliable transportation to travel to enter and function in the homes/facility of any patient/family in the service area to assess and provide patient care
• Must submit and pass all applicable pre-employment and annual tests and meet all training and other applicable state and federal guidelines
