About the role

Responsible for providing community-based case management services to individuals in need.

Responsibilities

  • Assess client needs and develop individualized service plans
  • Coordinate with community resources to provide comprehensive support
  • Maintain regular contact with clients to monitor progress and adjust plans as necessary
  • Document all interactions and progress in client files

Requirements

  • Bachelor’s degree in Social Work or related field
  • Minimum 2 years of relevant experience in case management or social work
  • Licenses/certifications in case management or social work preferred

Qualifications

  • Strong interpersonal and communication skills
  • Ability to work independently and as part of a team
  • Proficient in Microsoft Office Suite

Skills

  • Excellent organizational and time management skills
  • Knowledge of community resources and services
  • Ability to maintain confidentiality and handle sensitive information
