About the role
Responsible for providing community-based case management services to individuals in need.
Responsibilities
- Assess client needs and develop individualized service plans
- Coordinate with community resources to provide comprehensive support
- Maintain regular contact with clients to monitor progress and adjust plans as necessary
- Document all interactions and progress in client files
Requirements
- Bachelor’s degree in Social Work or related field
- Minimum 2 years of relevant experience in case management or social work
- Licenses/certifications in case management or social work preferred
Qualifications
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
- Proficient in Microsoft Office Suite
Skills
- Excellent organizational and time management skills
- Knowledge of community resources and services
- Ability to maintain confidentiality and handle sensitive information
