At Winding Waters Clinic, we are a Federally Qualified Health Center (FQHC) and a Patient-Centered Primary Care Home (PCPCH) committed to a team-based approach to care. We are seeking a Community Alignment Specialist to lead resource coordination, grow connections with local community-based organizations, and make a lasting impact on our public health mission.

Key Responsibilities

  • Partner with the Family Connects Oregon (FCO) nurse to identify family needs during home visits and connect them with resources.
  • Own the development of the FCO Wallowa County Provider Directory, a comprehensive registry of health and social services for local families.
  • Cultivate and strengthen partnerships with local healthcare providers, social service organizations, and community groups to align local resources.
  • Spearhead Wallowa County's FCO communications plan and support the Program Administrator in developing and running the Community Advisory Board (CAB).
  • Provide direct, one-on-one navigation support to help patients schedule appointments, understand care plans, and overcome barriers to care.
  • Screen patients for Health-Related Social Needs (HRSN) and act as a dedicated advocate to connect them with vital community resources and social services.
  • Serve as a critical link to our Medical-Legal Partnership (MLP) to identify patients with legal needs impacting their health.
  • Lead local outreach initiatives and deliver culturally appropriate health education to promote clinic services and public health programs.

Qualifications

  • A Certified Expert: Strong understanding of Community Health Worker (CHW) competencies and hold (or are committed to obtaining within 6 months of hire) an Oregon Traditional Health Worker certification.
  • A Master Communicator: Exceptional oral communication skills, utilizing motivational interviewing to build instant trust and rapport with families from highly diverse backgrounds.
  • A Tech-Savvy Advocate: Proficient in Electronic Health Records (EHR) systems, Microsoft Word, Microsoft Excel, and Google Docs. Understands how to leverage internet resources safely while strictly maintaining HIPAA compliance and data privacy.
  • Physical Resilience: Stamina to navigate long hours across both clinical spaces and mobile community outreach environments, including the manual dexterity to type/write and the ability to lift up to 35 pounds.

Preferred Experience & Attributes

  • Lived Experience: Personal experience successfully navigating maternal health, childbirth, lactation, and early childhood support systems.
  • Regional Knowledge: Familiarity with the social services, healthcare networks, and community resources available to families in Wallowa County or the surrounding region.
  • Peer Support: Desire or background in using your personal journey to mentor, advocate for, and empower local mothers.
