Responsibilities

  • Affords patient’s symptoms and creates customized treatment plan based on individual patient needs.
  • Screens patients for common behavioral health disorders using PHQ9, GAD7, Columbia Suicide Risk Assessment, or other tools as required.
  • Maintains monitoring of patients for changes in clinical symptoms and treatment side effects/complications, with a goal of reduction in symptoms.
  • Supports psychotropic medication management prescribed by PCPs, focusing on treatment adherence, side effects and other complications, and treatment effectiveness.
  • Tracks patient follow-up and clinical outcomes using a registry, and uses the system to identify and re-engage patients.
  • Facilitates care plan changes for patients who are not improving, including changes in medications or psychosocial treatments or appropriate referrals for additional services.
  • Manages behavioral health crisis as they arise in a primary care setting.
  • Provides flexible behavioral health services to patients on and off current caseload, both through scheduled appointments and immediate interventions via warm handoffs during medical visits with primary care providers.

Requirements

  • Master’s degree from an accredited institution in Social Work, Counseling, Marriage & Family Therapy, or related field.
  • A Mental Health Professional as defined within HB 1724 one of these higher credentials are held: Licensed Marriage & Family Therapist (LMFT), Licensed Mental Health Counselor (LMHC), SW Independent Clinical Lic (LICSW).
  • Minimum two (2) years of experience in a behavioral health setting.

About the Role

The Licensed Integrated Mental Health Therapist-BHN is responsible for independently providing assessments, treatment/care planning and psychosocial therapy within a primary care clinic setting for a diverse population of preventative, acute care and chronic disease management patients. The incumbent uses a variety of theoretical treatment models and interventions and excellent communication skills to work within a primary care/chronic disease management team setting with limited supervision; documents treatment/communication in keeping with department standards and keeps current on community resources to effectively provide linkage and advocacy to fulfill client’s needs.

Qualifications
