Collaborative Care Integrated Mental Health Therapist - Family Medicine NW Spokane
Responsibilities
- Affirms patient’s symptoms and creates customized treatment plan based on individual patient needs.
- Maintains monitoring of patients for changes in clinical symptoms and treatment side effects/complications, aiming for symptom reduction.
- Supports psychotropic medication management, focusing on adherence, side effects, complications, and treatment effectiveness.
- Tracks patient follow-up and clinical outcomes using a registry, and uses the system to identify and re-engage patients.
- Handles behavioral health crises in a primary care setting.
- Provides flexible behavioral health services to patients on and off current caseload, both through scheduled appointments and immediate interventions via warm handoffs during medical visits with primary care providers.
Requirements
- Master’s degree from an accredited institution in Social Work, Counseling, Marriage & Family Therapy, or related field.
- Mental Health Professional as defined within HB 1724, one of these higher credentials are held: Licensed Marriage & Family Therapist (LMFT), Licensed Mental Health Counselor (LMHC), SW Independent Clinical Lic (LICSW).
- Minimum two (2) years of experience in a behavioral health setting.
About the Role
The Integrated Mental Health Therapist (IMHT) works within a multidisciplinary team embedded in the primary care clinic and utilizes the Collaborative Care model of care. This team includes the IMHT, Psychiatric Consultant, & Primary Care Provider to provide comprehensive behavioral health services within the primary care setting.
Qualifications
Master’s degree from an accredited institution in Social Work, Counseling, Marriage & Family Therapy, or related field.
Mental Health Professional as defined within HB 1724, one of these higher credentials are held: Licensed Marriage & Family Therapist (LMFT), Licensed Mental Health Counselor (LMHC), SW Independent Clinical Lic (LICSW).
