Coder II

About the role

As a Coder II, you will ensure precise communication with insurance companies so that services are documented correctly and payments are processed efficiently. Every day you will accurately translate patients’ medical records into standardized codes for diagnoses and treatments. Using your expertise and training, you will ensure compliance with legal, regulatory, and organizational standards. Clear communication with providers and staff, combined with efficient management of records, ensures claims are processed correctly and on time.

Responsibilities

• Abstract, assign, and sequence ICD-10-CM/CPT/HCPCS codes to diagnoses and procedures as supported by documentation.
• Ensure the final diagnoses and operative procedures as stated by the physician are valid and coded to the highest level of specificity.
• Abstract all necessary information from documentation to identify secondary complications and co-morbid conditions.
• Meet FMG Production standards for coding procedures and FMG Quality standards per the Coding Audit and Monitoring process.
• Follow all Coding department policies and procedures.
• Understand and apply changes in the external regulatory environment and third-party reimbursement agencies, staying current with coding updates to ensure clean claims are submitted for adjudication.
• Perform a comprehensive review of documentation to ensure the presence of all component parts, such as patient and record identification, signatures, dates, and other necessary data.

Requirements

• Two years of coding experience using CPT and ICD-10-CM or equivalency.
• One of the following certifications upon hire:
  • Certified Coding Associate (CCA)
  • Certified Professional Coder (CPC)
  • Certified Professional Coder Apprentice (CPC-A)
  • Certified Coding Specialist (CCS)
  • Certified Coding Specialist - Physician Based (CCS-P)

$30.91 - $51.00 per hour
