Coder - Full Time
About the role
The Coder is responsible for timely review of patient records in order to identify appropriate codes, ensuring all data elements required for federal and state reporting are collected. They abstract records into the Hospital's computer system, assign appropriate ICD-10-CM, ICD-10-PCS, and CPT codes, sequence diagnoses and procedures, and write completed records to appropriate queues. Additional responsibilities include contacting physicians for additional information, assigning appropriate charge codes, and calling departments to obtain missing documentation.
Responsibilities
- Timely review of patient records to identify appropriate codes
- Collect all data elements required for federal and state reporting
- Abstract records into the Hospital's computer system
- Assign appropriate ICD-10-CM, ICD-10-PCS, and CPT codes
- Sequence diagnoses and procedures at least 98%
- Assign appropriate charge codes to Emergency Department encounters at least 98%
- Write completed records to appropriate queue at least 99%
- Call appropriate departments to obtain missing documentation
- Adhere to safety practices and report accidents/hazards
