About the role
At Easterseals PORT Health, we’re seeking a compassionate and dedicated full-time Community Support Team Co-Occurring Disorder Specialist to join our team in Raleigh, NC. As a valued member of our collaborative and supportive team, you will work together to transform the lives of individuals by providing a comprehensive range of services that treat the whole person and empower patients to find their way forward. The teams at Easterseals PORT Health care directly for our clients in the community, including visits to clients living in their own homes.
Responsibilities
- Provide services to individuals who have a mental health or substance use disorder and meet the two life domains.
- Conduct comprehensive substance abuse assessments.
- Use outreach and motivational interviewing techniques to work with individuals in earlier stages of change readiness.
- Facilitate the Person-Centered Planning process.
- Provide structured face-to-face interventions.
- Provide psychosocial rehabilitative interventions and case coordination.
- Carry out a full range of treatment functions wherever and whenever needed in a community setting.
- Work independently and be adept at problem solving and de-escalation in times of crisis.
- Participate in the crisis phone rotation.
Requirements
- Bachelor’s degree or Master’s degree in a human services field preferred.
- Must be certified as a CCS, LCAS, LCASA, CSAC or CADC.
- Able to be credentialed as a Mental Health Qualified Professional.
- Certification as a medical administrative technician or equivalent preferred.
- Valid driver’s license, current auto insurance and a good driving record.
- Demonstrate high moral and ethical character, and be free of any substantiated charges of abuse or neglect on the NC Healthcare Registry.

Schedule
Full-time position with a Monday–Friday flexible nontraditional schedule serving the Statesville and surrounding areas. This position is community based.
