Co-located Behavioral Health Technician - LADDER; Phoenix, AZ 85008 & 85029

About the role

Terros Health is hiring a Dually Located Behavioral Health Technician in the LADDER (Life Affirming Dual Diagnosis Education & Recovery) program to support both the McDowell Health Center and Desert Cove Health Center. This full-time position provides direct management of assigned behavioral health clients and program activities, facilitates teamwork, conducts recordkeeping, and makes appropriate referrals.

Responsibilities

• Provides transportation of clients in the Terros ATDT Program as needed; assists in planning transportation through external health partners when necessary; plans routes and facilitates this process.
• Contacts clients by phone to assist them in preparing for pickup in the mornings.
• Meets with clinical staff to offer observations/information regarding clients, including safety-related details.
• Conducts intakes and daily audits of new medical records; assists in maintaining quality and accuracy of medical records.
• Facilitates lunch time activity group daily and at least one group weekly; documents clinical intervention and client response.
• Facilitates leisure activities as needed or assigned, including program outings in the community.
• Participates in morning treatment meetings; contributes to client orientation, treatment planning, and client progress.
• Reports safety concerns promptly to clinical staff or program manager.
• Develops and maintains a resource log of discharge plans and activities; builds relationships with community agencies/services to ensure continuity of care.
• Maintains engagement with clients; follows up with case managers and other involved parties regarding referrals, scheduled intakes, and outreach attempts.
• Documents all coordination and outreach attempts.

Schedule

Full-time, Monday through Friday with flexible scheduling.
