About the Role

Provide routine daily nursing care and services to assigned residents in accordance with their assessment and care plan, under the direction of supervisors.

Responsibilities

  • Identify residents using wristbands or photo cards before administering treatments or serving meals.
  • Use only authorized facility abbreviations when recording information.
  • Report all changes in resident condition to the Nurse Supervisor/Charge Nurse promptly.
  • Record entries on flow sheets, notes, and charts in an informative and descriptive manner.
  • Report all accidents and incidents observed during the shift.
  • Maintain confidentiality of resident protected health information and report suspected violations.
  • Report unauthorized attempts to access the facility’s information system.
  • Perform only nursing care procedures for which you have been trained.
  • Prepare resident rooms for new admissions (e.g., made bed, name tags, admission kit).
  • Greet residents, escort them to their rooms, and introduce them to roommates and staff.
  • Inventory and mark resident personal possessions; assist with packing/unpacking during transfers or discharges.
  • Transport residents to new rooms or receiving areas; assist with loading/unloading residents to/from vehicles.
  • Maintain a warm, calm, and positive environment for residents.
  • Participate in shift meetings to identify and address problem areas or service improvements.
  • Report all resident complaints and grievances.
  • Follow established policies for exposure to blood/body fluids.
  • Perform routine tasks such as making beds, measuring vital signs (TPRs), and weighing residents.
  • Check on residents unable to call for help frequently; answer resident calls promptly.
  • Assist residents with meals, including identifying food arrangements and recording food/fluid intake.
  • Provide after-meal care (e.g., removing trays, cleaning residents).
  • Perform daily care functions (e.g., dental/mouth care, bathing, dressing, combing hair).
  • Keep residents dry by changing wet or soiled gowns, clothing, and linens.
  • Assist residents with medical tests, examinations, and restorative/rehabilitative procedures.
  • Lift, turn, move, position, and transport residents using proper techniques and equipment.
  • Observe and report pressure areas, skin breakdowns, and injuries of unknown origin.
  • Monitor disoriented and comatose residents; record and report data as instructed.
  • Perform special treatments as instructed.
  • Report missing residents immediately to the Nurse Supervisor/Charge Nurse.
  • Use only equipment you are trained to operate; report defective equipment.
  • Report allegations of resident abuse or misappropriation of property.
  • Honor resident refusal of treatment and report such requests to supervisors.
  • Cooperate with inter-departmental personnel to maintain adequate nursing services.
  • Notify the facility of lateness or absence from work.
  • Report occupational exposures to blood, body fluids, or hazardous materials per facility policies.

Requirements

  • Minimum 10th grade education.
  • Ability to read, analyze, and interpret professional documents, regulations, and procedures.
  • Ability to write reports, correspondence, and procedure manuals.
  • Ability to present information and respond to questions from managers and employees.
  • Basic mathematical skills (fractions, percentages, ratios, proportions).
  • Ability to solve practical problems in situations with limited standardization.
  • Ability to interpret instructions in written, oral, diagram, or schedule form.
  • Licensed Certified Nursing Assistant (CNA) in accordance with state laws.
  • CPR certification preferred.

Physical Demands

  • Occasionally lift and/or move up to 25 pounds.
  • Prolonged use of desktop or laptop computers.
  • Regularly required to sit, stand, walk, talk, read, and hear.
  • Frequent use of office equipment (copier, scanner, fax, telephone, calculator).
  • May assist in evacuating residents during emergencies.

Work Environment

The work environment is typically low to moderate noise. Reasonable accommodations may be made for individuals with disabilities.
