CNA Part-Time Evenings Memory Support

SPECIFIC DUTIES AND RESPONSIBILITIES

1. Report at start of shift with pocket care plans/worksheet to receive shift report.
2. Prioritize care with co-workers in assigned neighborhood based on resident needs.
3. Absorb regular attendance as an essential function.
4. Assist resident with the activities of daily living including but not limited to bathing, dressing, mouth care, toileting and transfers.
5. Responsible for appropriate documentation of activities.
6. Inform nurse of any findings or observations while caring for resident i.e. skin tears, bruises, accidents, incidents, change in status.
7. Ensure competency by completing state mandated 12 hours of in-services yearly.
8. Ensure competency in state mandated additional 6 hours of dementia training.
9. Awareness of safety hazards by maintaining a clean, neat and clutter free environment.
10. Maintain friendly, professional attitude with residents, visitors and staff.
11. Attend Resident Care Conference for neighborhood, meetings and in-services as required.
12. Assist with vital signs.
