About the role

The primary purpose of this position is to provide each assigned resident with routine daily nursing care and services in accordance with the resident's assessment and care plan, and as directed by supervisors.

Responsibilities

• Use wristband or photo card file to identify residents before administering treatments, serving meals, etc.
• Use only authorized abbreviations established by the facility when recording information.
• Report all changes in the resident's condition to the Nurse Supervisor/Charge Nurse as soon as practical.
• Record all entries on flow sheets, notes, charts, etc., in an informative, descriptive manner.
• Report all accidents and incidents observed on the shift that they occur.
• Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Administrator.
• Report any known or suspected unauthorized attempt to access the facility's information system.
• Perform only those nursing care procedures that you have been trained to do.
• Ensure that the resident's room is ready for receiving the resident (bed made, name tags up, admission kit available, etc.).
• Greet residents and escort them to their room; introduce resident to roommate and other residents and personnel as appropriate; make resident comfortable.
• Inventory and mark the resident's personal possessions as instructed; store resident's clothing.
• Assist residents with packing personal possessions when being transferred to a new room or discharged; transport residents to new rooms or to the receiving area; assist with loading/unloading residents to/from vehicles as necessary.
• Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift.
• Meet with shift nursing personnel on a regularly scheduled basis to assist in identifying and correcting problem areas and/or improvement of services.
• Report all complaints and grievances made by the resident.
• Participate in and receive the nursing report as instructed.
• Follow established policies concerning exposure to blood/body fluids.
• Make beds as instructed; put extra covers on beds as requested.
• Measure and record temperatures, pulses, and respirations (TPRs) of residents as instructed; weigh and measure residents as instructed.
• Ensure that residents who are unable to call for help are checked frequently.
• Answer resident calls promptly.
• Check residents routinely to ensure that their personal care needs are being met.
• Assist residents with identifying food arrangements (informing resident with sight problems of foods on tray, location, temperature, etc.).
• Record the resident's food/fluid intake; report changes in eating habits to the Nurse Supervisor/Charge Nurse.
• Keep residents' water pitchers clean and filled with fresh water on each shift, within easy reach of the resident.
• Perform after meal care (remove trays, clean resident's hands, face, clothing, etc.).
• Participate in and receive the nursing report upon reporting for duty.
• Perform all assigned tasks in accordance with established policies and procedures and as instructed by supervisors.
• Follow work assignments and/or work schedules in completing and performing assigned tasks.
• Cooperate with inter-departmental personnel and other facility personnel to ensure nursing services can be adequately maintained to meet residents' needs.
• Notify the facility when late or absent from work.
• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with facility policies and procedures.
• Assist residents with daily functions (dental and mouth care, bath functions, combing of hair, dressing and undressing as necessary).
• Keep residents dry (change gown, clothing and linens when wet or soiled).
• Assist residents in preparing for medical tests (lab work, x-ray, therapy, dental, etc.).
• Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
• Perform restorative and rehabilitative procedures as instructed.
• Assist in preparing the resident for a physical examination.
• Check each resident routinely to ensure personal care needs are being met in accordance with their wishes.
• Observe and report the presence of pressure areas and skin breakdowns to prevent decubitus ulcers (bedsores).
• Report injuries of an unknown source, including skin tears.
• Observe disoriented and comatose residents; record and report data as instructed.
• Perform special treatments as instructed.
• Immediately notify the Nurse Supervisor/Charge Nurse of any resident leaving/missing from the facility.
• Use only equipment you have been trained to use; report defective equipment to the Nurse Supervisor/Charge Nurse.
• Maintain the confidentiality of all resident care information including protected health information; report known or suspected incidents of unauthorized disclosure.
• Report all allegations of resident abuse and/or misappropriation of resident property.
• Honor the resident's refusal of treatment request and report such requests to your supervisor.

Requirements

• Must possess, as a minimum, a 10th grade education.
• Must be a licensed Certified Nursing Assistant in accordance with laws of the state.

Qualifications

• Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
• Ability to write reports, business correspondence, and procedure manuals.
• Ability to effectively present information and respond to questions from managers and employees.
• Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
• Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
• Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
• CPR preferred.

Physical Demands

• Must occasionally lift and/or move up to 25 pounds.
• Prolonged use of a desktop or laptop computer.
• Regularly required to sit, stand, walk, talk, read, or hear.
• Frequent use of office related equipment including copier/scanner/fax, telephone, and calculator.
• May be necessary to assist in the evacuation of residents during emergency situations.
