About the Role

Provide routine daily nursing care and services to assigned residents in accordance with their assessment and care plan, under the direction of supervisors.

Responsibilities

• Identify residents using wristbands or photo cards before administering treatments or serving meals.
• Use only authorized facility abbreviations when recording information.
• Report all changes in resident condition to the Nurse Supervisor/Charge Nurse promptly.
• Record entries on flow sheets, notes, and charts in an informative, descriptive manner.
• Report all accidents and incidents observed during the shift.
• Maintain confidentiality of resident’s protected health information and report suspected violations.
• Report unauthorized attempts to access the facility’s information system.
• Perform only nursing care procedures for which you have been trained.
• Prepare resident rooms for new admissions (e.g., made beds, name tags, admission kits).
• Greet residents, escort them to their rooms, and introduce them to roommates and staff.
• Inventory and mark residents’ personal possessions as instructed.
• Assist residents with packing/unpacking personal possessions during room transfers or discharges.
• Transport residents to new rooms or receiving areas and assist with loading/unloading from vehicles.
• Maintain a warm, calm, and positive environment for residents.
• Meet regularly with nursing personnel to identify and correct problem areas or improve services.
• Report all resident complaints and grievances.
• Participate in and receive nursing reports as instructed.
• Follow established policies for exposure to blood/body fluids.
• Make and prepare beds, including adding extra covers as requested.
• Measure and record temperatures, pulses, respirations (TPRs), and weights as instructed.
• Frequently check residents unable to call for help.
• Answer resident calls promptly and ensure personal care needs are met.
• Assist residents with food arrangements and record food/fluid intake.
• Report changes in residents’ eating habits to the Nurse Supervisor/Charge Nurse.
• Keep residents’ water pitchers clean, filled, and within easy reach.
• Perform after-meal care (e.g., remove trays, clean residents’ hands, face, and clothing).
• Perform all assigned tasks according to established policies and procedures.
• Follow work assignments and schedules to complete tasks.
• Cooperate with inter-departmental personnel to maintain nursing services.
• Notify the facility of lateness or absence from work.
• Report occupational exposures to blood, body fluids, or hazardous chemicals per facility policies.
• Assist residents with daily functions (e.g., dental/mouth care, bathing, dressing, combing hair).
• Keep residents dry by changing gowns, clothing, and linens when wet or soiled.
• Assist residents in preparing for medical tests (e.g., lab work, x-rays, therapy).
• Assist with lifting, turning, moving, positioning, and transporting residents.
• Perform restorative and rehabilitative procedures as instructed.
• Prepare residents for physical examinations.
• Check residents routinely to ensure personal care needs are met.
• Observe and report pressure areas, skin breakdowns, or injuries of unknown source.
• Observe disoriented and comatose residents and report data as instructed.
• Perform special treatments as instructed.
• Report any resident leaving or missing from the facility immediately.
• Use only equipment you have been trained to operate and report defective equipment.
• Maintain confidentiality of all resident care information and report unauthorized disclosures.
• Report allegations of resident abuse or misappropriation of property.
• Honor residents’ refusal of treatment and report such requests to supervisors.

Requirements

• Minimum 10th-grade education.
• Ability to read, analyze, and interpret professional journals, technical procedures, or regulations.
• Ability to write reports, business correspondence, and procedure manuals.
• Ability to present information and respond to questions from managers and employees.
• Ability to apply mathematical concepts (fractions, percentages, ratios) to practical situations.
• Ability to solve practical problems with limited standardization.
• Ability to interpret instructions in written, oral, diagram, or schedule form.
• Licensed Certified Nursing Assistant (CNA) in accordance with state laws.
• CPR certification preferred.

Physical Demands

• Occasionally lift and/or move up to 25 pounds.
• Prolonged use of desktop or laptop computers.
• Regularly required to sit, stand, walk, talk, read, or hear.
• Frequent use of office equipment (copier, scanner, fax, telephone, calculator).
• May assist in evacuating residents during emergencies.

Work Environment

• Low to moderate noise levels.
